There are two convenient ways to register for the class you choose:

1. Print the flyer for the class, complete the form and fax it in.
   
2. Email   Click this email link which includes a registration form. 
   Using email or fax, your registration will be confirmed by email and your MADA Services Account will be billed directly.  Confirmations and directions will be sent prior to the class.  Refunds are available only for cancellations 48 hours in advance of class.
